Summary
Non-obstructing stone causing inflammation of gallbladder
RUQ pain (Murphy's sign) + Sepsis
- USS
- IV Abx
- Early laparoscopic cholecystectomy
Definition
Inflammation of the gallbladder
Pathophysiology
- 90% are secondary to gallstones but not causing obstructions
Presentation

RUQ pain +/- radiation to Rt shoulder

Fever / Sepsis
-
Murphy's sign - inspiratory arrest on RUQ palpation
-
LFT is typically normal (non-obstructing stone)
- cholestatic LFT → consider Mirizzi syndrome
- Gallstone impacted in distal cystic duct
Workup
- Ultrasound first line
- Cholescintigraphy (HIDA scan) may be indicated if USS inconclusive
Management
- IV antibiotics
- Early laparoscopic cholecystectomy within 1 week of diagnosis
